Minority Teachers of Illinois (MTI) Scholarship Program Application/Teaching Agreement/Promissory Note

Welcome to the MTI Program Application.

Be sure to read the important information here, and on our website, about the MTI program and the teaching obligation connected to the award before you complete the application.

If you have a Student Portal profile, some of the application information will be prepopulated for you if you sign in before starting the application. If you need to create a profile, access the Student Portal and click on Login.

As part of the application process, you must agree to the terms and conditions outlined in the applications Teaching Agreement/Promissory Note and in the Program Requirements area of the ISAC website.

Teaching Obligation

If you are an MTI Scholarship recipient, you must teach on a full-time basis for each academic year, or portion of an academic year, in which you received the award. This teaching obligation must be fulfilled at a nonprofit Illinois public, private or parochial preschool, elementary or secondary school at which no less than 30 percent of the enrolled students are minority students, as certified by the Illinois State Board of Education (ISBE). You must begin teaching within one year following termination from the program of study funded by the scholarship, and continue until the teaching obligation is fulfilled. If the teaching commitment is not fulfilled, the scholarship converts to a loan, and you must repay the entire amount, plus interest.

Completing the Application

Read and follow all instructions and prompts as you answer the questions. You must agree to the program's Terms and Conditions before proceeding through the application. Providing your e-mail address is optional, but by doing so you will receive application status updates.

The data you input here will be automatically loaded into ISAC's system, however, your application will not be reviewed or processed until a printed and signed (original ink signature only) paper application is received in ISAC's Deerfield office.

After printing the complete application packet, submit only the one-page application; do not include the pages containing the Terms and Conditions. Be sure to retain a copy of your signed Application/Teaching Agreement/Promissory Note, along with the Terms and Conditions, for your records.

Only paper applications with original signatures will be accepted; do not submit the application via e-mail or FAX. For priority consideration, applications must be postmarked on or before March 1 preceding the academic year for which you are applying.

Please note the MTI application cannot be completed through your mobile or tablet device. You must use a Wi-fi-enabled desktop or laptop, connected to a printer, to complete the application.
